Description
This module allows clients who may want visitors to the site to fill out their demographic information in exchange for accessing or downloading information. This module is particularly helpful when it comes to the downloading of media kits, entering contests, or any interaction whereas a user needs to fill out information first, in order to receive information from the site.
This module allows you to manage the presentation of the form, including top copy (why you are asking for the information), field validation (which fields a user needs to fill in in which to get it), the Results text (information which thanks the user for submitting their request), File Upload (allowing you to upload the information that the user, in turn is able to download when completed), and even email subject and recipient information, which forwards all of these incoming requests to the right individual within your organization. All submissions made, triage email messages to the right people in your company, immediately, and further store all activity in a Reporting module (Reporting/Contact Forms), to allow you to export the total list of activity per request at any time in the future.
For an example, Publishers often want to make their Media Kit (Advertising prices) available to prospective advertisers. In this example, a site visitor can fill out their information, in which to download the Media Kit. As soon as they fill out the form, which takes them to a thank you page, offering a link for download, emails are sent to the assigned recipients within your organization, and the transaction is saved in the database...giving you an audit trail of exactly 'who' has requested 'what' information 'when'; giving your team the ability to respond quickly to the needs of online consumers.
How To Use
This module is found within the Forms tab of your Client Console. To create a new form, simply click on new. Within the Top Copy, simply create some information which explains why you are asking for the user's information. The Results Text, allows you to manage the thank you message that the user sees, after filling out the form, File Upload and URL, allows you to either present a link that the user will gain access to (if another form), or gives you the ability to upload a file (up to 620 file types are supported, and often represents a PDF, Video file, or Word Document). You will also be able to validate which fields are required, and which ones show on the site, and where in the site, this form will live (Relative to which site navigation section).

Tips About This Module
Once you have turned on a contact or info request form within your site, you can always get the link of this form (from the front of the site, under its respective site navigation area), and re-link it wherever you may want (from an advertising record, or 'within' a page in the site). Simply copy and paste the link of the form, and then go back into your record (Within the Client Console) and either place it on hold, or Select 'No' within the 'View on Site' attribute. This will allow you to have a portable form which can be managed anywhere.
This tool is often used in relationship to email broadcast campaigns, in which the links available in the email sent actually link to one of these forms. In this way, you can track incoming lead activity and save them to a database, for all outbound emails you send....giving you insite into who is reading them, or who has downloaded online videos, tutorials or files which have been requested.
This module is also often used as a 'Guestbook' for site visitors, and is a great way to collect potential client information, without forcing them into full blown site 'membership'
